British nationals wishing to sign A Letter of Assurance /Zapewnienie malzenskie/ can do so at the Consular Section of the Embassy of the Republic of Poland in London. The forms (available at the Consular Section) need to be filled in and signed personally with a valid passport. The consul fee is GBP 30,-
